How it works.

  • Give permission and verify your identity.

This allows us to begin requesting your records from your providers and start building your health story.

  • Status message indicating 'In progress' with icons for email, search, and protection, and a message about reaching out to healthcare providers for health records.

We start retrieving your health records.

Records may come from different sources. This can take time, depending on where your information is held.

  • Screenshot of a healthcare request management interface showing a list of healthcare organizations and professionals, with an option to view archived requests and a blue 'New request' button.

A health summary is created and reviewed.

This summarizes key information in a clear format that is reviewed by a Medda Care physician for accuracy.

  • Access your records and summary in one place.

As records arrive, you can see what’s available, what’s pending, and what may be missing.

For when you…

  • When you’re not sure what information exists until you’re already in the room.

  • When conversations move quickly, and it’s hard to remember key details.

  • When your history lives in different places and you’re tired of repeating the same story.

  • When it’s unclear which information is on file, what’s missing, or where things are held.

  • When records feel technical or confusing, and you’ve turned to online searches or AI tools for clarity.

What to expect.

Healthcare records are not stored in a single location. As your health summary is being built, records may arrive at different times or may be unavailable, depending on where they’re held and how they’re released.

Your health summary is based on the records received at the time it is created. If some information has not arrived, your summary may not include everything.

Have questions?

  • This helps us know where to request your records from and improves retrieval accuracy. If you don’t have a primary care provider, that’s okay. We can still retrieve available information.

  • Your health card number helps ensure the correct records are matched to you and reduces the risk of errors during retrieval.

  • Identity verification helps ensure the correct records are matched to you and reduces the risk of errors during retrieval.

  • Yes. Your information is handled securely and only accessed with your permission.

  • Not always. Your health summary is based on the records received at the time it is created. If some records are delayed or unavailable, they may not be included.

  • No. The health summary does not diagnose, assess, or provide medical advice.

It organizes information from your existing health records into a clear, easy-to-read overview. It’s designed to help you understand what information exists and bring it into your care conversations with your providers.
